Enzyme Injection Shows Promise For Reducing Spinal Surgery In a familiar psychology test, students are asked to describe in words an eight-ounce glass that contains four ounces of liquid. The optimists say the glass is half full: the pessimists say it is half empty. In the same way, bridge players sort themselves out as either optimists or pessimists when a high trump drops. The scene was a match between England and Wales, not long ago. At both tables the final contract and opening lead were the same.

The procedure was developed principally by Dr.

Lyman Smith, a member of the Northwestern University medical faculty. The injected enzyme dissolves the protruding portion of the disk, making surgery unnecessary in some cases. So far, about 7,000 patients have been treated with injections of the enzyme, which is related to meat tenderizers derived from papaya. Thirty-five U.S. and Canadian investigators have been authorized to use it.

So far, the investigators a had surgery I probably wouldn't have played again," he is quoted as having said. "It's been great, a fantastic success." So far, the investigators are reporting about 90 per cent good to excellent results with this chemical, first developed more than nine years ago, in the treatment of ruptured disks. Two athletes have been among those who have reported successful treatment with the enzyme. One is Bill Melton, conditions for which fusion is sometimes required. Among them is degenerative arthritis.

This occurs in everybody, starting in middle age, and does not cause any problem in most people. When it does, it often can be treated successfully with aspi-. rin and exercise and only occasionally requires fusion. Fusion also is done for the so-called "slipped disk" or "ruptured disk." Meyer said that perhaps only 2 per cent of those who think their backache is caused by this problem actually have it. When it does happen, the fibrous, jellylike pad which separates the vertebrae slips or is forced out of its place.

Pain results when this extruded mass causes pressure on a nerve. It is this problem which chymopapain has been shown to treat successfully. Another spinal operation, laminectomy, sometimes is used to relieve the pressure which causes pain. Meyer described this as "unroofing of a disk or nerve root." The lamina, part of the vertebra, is removed in this procedure. However, spinal fusion sometimes is later necessary when this does not work.

Both Wiltse and Meyer said that backache often is a psychosomatic complaint, caused by emotional problems. But it still hurts just as much. And both orthopedists cited the special problem of older heavy-duty workers who have back fusion. The back is never restored 100 per cent, and these workers often cannot return to their former jobs. Meyer said improved surgical techniques and more meticulous surgery have helped make the fusion operation more successful and the length of time in bed shorter.

Generally, two or three months are required for a fusion to heal completely if it involves one or two segments of the spine, Meyer said, but the patient may be out of bed in two or three days. If the fusion is more extensive, as in scoliosis, the patient may be in bed six to 12 weeks, followed by six months in a cast, though up and walking. Orthopedic specialists are reluctant to talk about chymopapain because of FDA regu- and promptly led out the ace of hearts. When West dropped the jack of hearts, our English friend continued with the king of hearts, hoping to capture the queen of hearts too. This optimistic play sealed South 's doom.

East was now sure to win two trump tricks, just enough to defeat the contract. AVERTS DISASTER The Welsh declarer observed the same facts but took a pessimistic view. After the first two tricks, he worried about the trump situation. Instead of leading out the king of hearts, he led a low trump. Dummy's ten lost to East's queen.

East returned a club and West took two tricks in that suit. South ruffed the next club and got to dummy with a diamond to return a trump. South had the K-8 over East's 9-7, so he was able to draw the trumps without further loss. He then had enough top cards to take the rest of the tricks. The first declarer's optimistic play would have been justified if his contract had been five hearts since then he would need good luck with the trumps to make his rather foolish con- mm By CO.

MCDAMEL AP Science Writer CHICAGO (AP) About a quarter of all Americans have bad enough backaches to go to a doctor about them. For a small fraction, surgery is required, sometimes involving fusion of a portion of the spine. Over the past few decades, the time in bed required has dropped for some patients, to a day or so, compared with the two or three weeks or even months they formerly were kept down. The time still varies, however. Those who require more extensive fusion involving several vertebrae have, of course, to stay in bed longer.

(AP) For a man who never studied law, Leo Altmix has made somewhat of a splash in the judiciary. Altmix, 56, recently was formally certified as a member of the Adams County Bar Association. Officials say there probably are not more than three non-lawyers in the state who are members of a bar association. Altmix has been a judge for 12 years. A former butcher and meat truck driver, Altmix became a Quincy alderman in 1955, a post he held until being elected justice of the peace in 1961.

Subsequent reorganizations of the Illinois judiciary elevated him first to a magistrate and now to associate judge in Quincy 's Circuit Court system. Babies Switched YAMAGATA. It is being used in only a few medical centers, pending further approval by the Food and Drug Administration before it is generally available. Dr. Leon L. Wiltse, a faculty member at the University of California Medical School at Irvine and staff member at Long Beach Hospital, said in an interview that orthopedic surgeons try to treat back aches conservatively, trying to relieve the pain without turning to surgery. "We're not fusing nearly as many spines as formerly," Wiltse said, although he added that some orthopedic surgeons might disagree.

Fusion involves transplanting part of the ilium found in the hip bone, or of the tibia, the calf bone, to the spine to stabilize a diseased or injured portion. Artificial devices, such as metal rods or plates and springs, also may be used. Dr. Paul R. Meyer Jr.

a member of the Northwestern Unviersity Medical School faculty and director of the spinal cord injury unit at North-w western Memorial Hospital, said spinal fusion today is most commonly used to correct idiopathic scoliosis curvature of the spine from an unknown cause. This occurs most often in young women. Scoliosis at one time also was assocaiated with polio, but that disaease has been virtually wiped out. Meyer outlined several other I Answers For Veterans A-Q. My brother became addicted to heroin but came out of the service with a good discharge.

His private clinic just went out of business because the methadone program was slipping. Would the Veterans Administration put him back on methadone or let him go drug-free? A If he appears able to make it in a drug-free program, VA would let him try that. If he failed to remain drug free, he could receive methadone at a VA clinic. I am a female veteran receiving nonservice-connected pension. Can I claim my husband as a dependent? A Yes.

Contact your nearest Veterans Administration office for proper guidance. Veterans organization representatives and county service officers (if available in your state) can also provide information. My husband has not changed his beneficiary on his national service Life Insurance policy since our marriage. He says it is not necessary as the insurance is automatically paid to the wife. Is this true? A This is not true.

Insurance proceeds are paid to the beneficiary last designated in writing by the insured. If none is designated, or no designated beneficiary survives the insured, proceeds will be paid to the estate. New Bicycle "No-No" Sign An increasing number of bicycles are entering the traffic stream on our highways. In some cases, this poses serious problems in safely combining a mixture of bicycle and motor vehicle traffic. As a result, many plans for roads that will be built in the near future will include parallel bicycle trails or lanes.

Additionally, more and more facilities are being provided for recreational bicycling. Special signs will be provided to guide cyclists to specific bicycle routes and a new symbol sign has been developed for areas where bicycle use is prohibited. The sign depicts a bicycle with a red circle and slash superimposed over it. This practice of using a red circle and slash to indicate a prohibited movement is also used in Europe and Canada. It's a shame that it has finally come to this. But, retailers can no longer be soft on shoplifters. Today, food stores are losing one billion dollars a year through pilferage.

It is not unusual for a single supermarket to lose $15,000 to a year through theft. Retailers cannot absorb the total loss.
